multiple copies of a plugin get added

for some reason (that i can’t find a consistent cause) when adding plugins from the menu sometimes multiple copies are added (eg click c* ampV and i sometimes get up to 5 or 6 copies of it added instead of one). this doesn’t happen all the time and i can’t find a consistent cause of it. any ideas as to what it is?

(using 2.0 branch svn build 3366)
